Benthos RMA Form

Customer must complete the form with ALL required information. Incomplete forms may delay processing.  After submitting this form you will recieve an email confirmation of the submission. After the request is reviewed by Teledyne Benthos Customer Service you will receive​  a summary of the request and a Return Authorization Number which must be placed on the outside of the box and referenced on the shipping documentation.

You will also be provided shipping instructions. Please do not ship return items without first recieving the shipping instructions and RMA number.​
